Support Staffordshire is working with a number of Primary Care Networks (PCN) across Staffordshire to deliver a Social Prescribing service to patients

What is Social Prescribing?

Social Prescribing is a way of enabling GPs and other healthcare professionals to connect patients to community activities, networks and non - clinical services that can help to meet their social, emotional or practical needs; with the help of a Link Worker

Why Social Prescribing?

Social Prescribing can help with things that cannot be fixed by doctors and medicine alone. If you have a problem that is not medical, it can be a way of helping you to connect to the most appropriate support. It can help you to find ways to fell more on control and part of your community.

How can Social Prescribing help you?

Your Link Worker will talk to you to understand your needs and then discuss what available help and support there is.  You can gain more control over your own health and wellbeing and find ways to improve how you feel.

Examples of Social Prescriptions:

•Connecting with Nature Training
•Mindfulness
•Gardening
•Employment Advice
•Benefits Advice

•Social Care

•Mental Health Support

•Peer Support

•Social-Care

•Dementia-support

What is social prescribing?

This is when you are referred or signposted into non-clinical services by a professional as a way of supporting or improving your health and wellbeing.

Social Prescribing Link Workers are new posts, funded by the NHS, who will support you and make referrals on your behalf into community services.

Do you want to get involved in your community and help others?

There are hundreds of local community organisations registered that you can help by volunteering.

Maybe you want to work with animals, help the homeless, or get involved in a fundraising event - whatever it is, there is someone out there who needs your help.
